Apply to Become a Mentor

Thank you for your interest in becoming an ISYFAMentor. This form has two parts: the first part (see below) is to establish your account to log into the ISYFAMentor microsite. The second part will ask you to enter professional profile information, including a profile photograph and your resume. Please have these documents ready to make the application process smoother. In the event that your session expires (i.e., spend longer than 10 minutes filling out the form) you will need to restart the application process. Once you successfully reach the confirmation page, your application will be sent to the ISYFAMentor Committee. Please allow a minimum of 5 business days for us to review your application.

As a mentor within the ISYFAMentor community, I agree to:
- Commit 3 months to each mentoring relationship (4 hours per month for mentors and 6 hours per month for mentees).
- Not promote goods or services from sources in which I have a financial or personal interest.
- Not to accept fees or commissions.
- Encourage my mentee to maintain the highest ethical standards. 
- Maintain a high standard of personal and professional conduct with my mentee. 
- Always communicate in a truthful manner.
- Support the goals of my mentee.
- Avoid the intent or appearance of unethical or compromising practice in relationships, actions and communications. <br />
- Accept only assignments for which I am qualified by my experience and competence.
- Refer my mentee to ISYFAMentor for issues or questions for which I feel unqualified.
